Title: Enhancing HIV Care Continuum for People Who Inject Drugs (PWID) in LMIC Settings: Overcoming Post-COVID Challenges with Iris Biometric Identification

1. Introduction
The POINTER study is a longitudinal investigation focused on improving the "treat and retain" stages of the HIV care continuum among PWID in low- and middle-income countries (LMICs). A critical aspect of the study involves accurate and reliable participant identification for enrolment and ongoing data collection.

2. Background
Prior to the COVID-19 pandemic, fingerprint scanners were the primary method for participant identification in the POINTER study. However, the pandemic heightened concerns about potential infection transmission through shared surfaces, leading to widespread reluctance among PWID to utilize fingerprint scanners.

3. The Challenge
The study faced the critical challenge of finding a robust and hygienic alternative to fingerprint scanning for participant identification. This was particularly crucial as accurate identification is paramount for:
Enrolment: Ensuring accurate demographic and baseline data collection for each participant.
Data Integrity: Maintaining consistent data records throughout the study period.
Participant Tracking: Monitoring participant engagement in HIV care and treatment adherence.

4. Solution
To address this challenge, the research team developed a novel solution:
Iris Biometric Integration: Iris recognition technology was selected as a highly accurate and hygienic alternative to fingerprint scanning.
Custom Android Application: A dedicated Android application was developed to capture high-quality iris images of participants.
ODK-X Integration: The iris capture application was seamlessly integrated with the existing ODK-X-based data capture system used in the POINTER study. This ensured smooth data flow and consistency across the study.
Tablet Deployment: Android tablets were equipped with both the iris capture application and the ODK-X data capture system, providing a user-friendly and efficient platform for data collection.

5. Implementation
The iris biometric system was rigorously tested for accuracy and reliability before implementation. Training was provided to research staff on the proper use of the iris capture application and data entry procedures. The system was then deployed in the field, replacing fingerprint scanning as the primary method of participant identification.

6. Results
Enhanced Participant Acceptance: The adoption of iris biometrics significantly improved participant acceptance and engagement in the study. Concerns about infection transmission were significantly reduced.
Improved Data Accuracy: The use of iris recognition technology enhanced data accuracy and reduced the risk of data entry errors associated with manual identification methods.
Streamlined Data Collection: The integration of the iris capture application with the ODK-X system streamlined the data collection process, improving efficiency and reducing the burden on research staff.
Increased Data Security: Iris biometrics provide a highly secure and tamper-proof method of participant identification, enhancing the overall security and integrity of the study data.

7. Conclusion
The successful integration of iris biometrics into the POINTER study demonstrates the feasibility and effectiveness of using this technology to overcome challenges in participant identification within HIV research in LMIC settings. This innovative approach has significant implications for future research endeavors, particularly those involving vulnerable populations and requiring robust and hygienic identification methods.

This case study highlights the importance of adapting research methodologies to address emerging challenges and the potential of innovative technologies to improve the quality and efficiency of HIV research in resource-limited settings.
